Safe Use of Your Car Jack

Every new passenger vehicle sold today comes with a jack for use when lifting is required, usually to change a tyre or mechanical repairs. We all know how a jack works but do we all know how to use a jack safely?

The Facts

Between 1989 & 1992, twenty seven Australians lost their lives from vehicles falling off jacks and ramps that were suppose to be supporting them. Interestingly, while 20 of the victims were home users, 7 were killed while in a place of work, where safe procedures should be known & practiced.

Why did this happen? In the cases where the cause was identifiable, it was discovered the users had:

1 Failed to apply the hand break on the vehicle.

2 Failed to leave the vehicle in gear.

3 Not used chocks on the other wheels.

4 Incorrectly used the jack.

5 Used an inappropriate jack for the job.

6 Attempted to jack the vehicle on sloping or uneven ground.

By ignoring these simple procedures the unfortunate victims paid with their lives.

Safe Use

Always read the instructions that come with your jack & follow them to the letter.

1 Its a very bad idea to stand in the road while using your jack, however some times it is unavoidable. If you must do this be sure to light your workspace as much as possible so on coming traffic can see you. Flares & reflectors are best for this if available. Try to have someone standing beside you facing the traffic & directing it clear if needed.

2 Always work on a flat, solid surface. Most jacks do not function safely on uneven or soft surfaces. Air jacks like Selson are the exception to this rule. Its broad base & internal telescopic arm allow the jack to follow the lifting arc of the vehicle on all surfaces & gentle slopes.

3 Be sure no one remains in the vehicle. The smallest shake can unsettle the jack. Remove any heavy loads from inside the vehicle.

4 Apply the handbrake and leave your vehicle in gear.

5 Use chocks on the non-raised wheels.

6 Never place any part of your body under the vehicle.

7 Never raise the vehicle any higher than necessary.

8 Always keep an eye on the jack to see if its holding steady. If it looks to be slipping or moving in any way, get away from the vehicle.

Are all jacks equal?

Most cars come with a pentagram or scissor jack. These are adequate for lift about 1/3rd of the weight of the vehicle. As you are only lifting one corner, this is acceptable. In a garage or workshop situation bottle or hydraulic jacks are common. The later in particular can lift heavier weights than scissor jacks and are considered safer. We all have enough "In memory of" patches on our vest and do not wish to add any more. So here are 11 motorcycle riding tips to remind us of the things that can keep us in one piece and out of the hospital.

If you are a novice, sign up for and attend a Motorcycle Safety Foundation course. Not only will this course teach you lots of the things that will help keep you safe but your motorcycle insurance will probably be slightly lower as well and who doesn't want to save on insurance, especially when riding in states with Helmet Optional laws that require large bodily injury coverage to enjoy the right for those who ride to decide.

Remind yourself where your suicide or kill switch is located. This switch, if you realize you are going to have to drop the bike, can save you from having your own motorcycle run over you causing injury.

If you are just learning to ride and live in a highly populated area, have an experienced rider take you and your bike into an area with little or no traffic. A parking lot of a closed business works very well. Neighborhood streets can cause the residents to complain. Ride in this controlled environment until you feel completely in control of the vehicle. Learn how to stop and start easily and smoothly. Learn how long it takes to stop safely it isn't the same as stopping a car.

First-time street riders should choose a time of day when the traffic is at a low. Jumping into rush hour traffic can easily lead to an accident on your very first outing. Select a route that takes you into very low traffic zones such as through neighborhoods, scenic routes and avoid at all cost getting on a freeway until you gain quite a lot of experience.

Never, ever become aggressive toward a car or truck driver. Those cars win every single time in a road rage situation. They are bigger, heavier and have a lot more power. If someone tailgates you, tap your brake lights a few times and if they don't back off, change lanes or give them the road by pulling off to the side

We all know black is the only cool biker color, right? Well, if you are going to be riding at night in a poorly light area, it is much more important to be seen than to be cool, no matter what your friends say! Be sure your lights are bright and clean for maximum visibility but also consider placing reflective tape on the back of your helmet (if you use one) and maybe a glow in the dark patch on your back.

Never trust cages. Car drivers somehow, no matter how loud or how rowdy bikers can be, manage to simply not see us! The blind spot in most cars is sufficiently large for a motorcycle to be completely hidden from view. But other cage drivers simply don't seem to look for us. Unless, of course, they son, daughter, husband or wife happens to ride; unfortunately, we can't tell who those people are and have to be doubly aware of other vehicles than when driving a car.

Never assume that just because there is no turn signal on that car that it isn't going to turn right in front of you. No matter how many other drivers don't use turn signals, ALWAYS use yours and, if you have a passenger during daylight, hand signals can be an extra preventative measure.

You know those trucker bumper stickers that say "If you can't see my mirrors, I can't see you"? Well, use this same common sense when riding. If you can't see the driver's face in the rear view mirror of the vehicle in front of you, chances are they can't see you at all. Stay out of blind spots at all times.

Once you become an experienced rider, when riding on city streets, always allow room for other people's errors. An added safety tip is that you should remain aware of where you would choose to put the scoot down if the need arose. If you get in trouble and have any time at all, select where to lay the motorcycle instead of allowing traffic or the bike to determine that for you. If you can elect to place the bike on the ground, allowing it to move out from between your legs at low speed, you will experience much less road rash than allowing momentum to take you with the motorcycle.

Novice riders should never carry passengers. That comes later with more road experience. The movements of a rider can cause a situation where you can easily lose control, especially at very low speeds.

Above all, plan for safety so you will come home safely to ride again another day. Road rash and accidents costing hundreds of thousands in medical bills are all too common. Be defensive and be aware to avoid losing your bike or worse, your life.

What Is MCSE Certification?

Studies show that people who work in the IT industry are paid much more than others. This is probably because it is a specialized field to get into and you have to study and get certified to work among the best.

One of the most prestigious ones around is called MCSE certification. Those who pass are called Microsoft Certified Systems Engineers and only those that have been working in the IT industry for about a year are allowed to enroll in the program.

There are two types of MCSE's currently available. The first is for Windows 2000 and the other is for Windows 2003.

Those that decide to take the first one will have to deal with exams such as 70-210 Windows 2000 Professional, 70-215 Windows 2000 Server, 70-216 Network Administration, 70-219 Active Directory Design and elective exams such as 70-220 as well as 70-224 Exchange 2000 Server.

The second one which is Windows 2003 has an exam called 70-290, which is supposed to test your mental ability when it comes to managing and maintaining a Microsoft Windows Server 2003. There is also 70-291 that is an environment exam and 70-294 that tackles tests planning.

Anyone that wants to become certified will have to undergo IT training before taking the exam. Such places are called boot camps and after paying between $4,000 to $6,000 for a course that will last 2 weeks, you will be given all the review material needed such as practice tests with questions and answers to help you understand key concepts, tutorials, study notes and other tips. Some of these learning centers will even offer a money back guarantee if you don't pass on the first try.

If the review material is not enough, you can even have someone tutor you along the way. This person will be there to determine your strong points so more focus can be given on topics that you need a lot of improvement. Those that can review on their own can buy the book that already comes with a CD, which costs about $1,000.

In order to get MCSE certification, you have to pass a battery of exams. Five of which are operating systems while the other 2 are electives.

Given that the cost of the program is quite expensive, you can try to ask your employer if they are willing to pay for it. Some companies do that in exchange for a number of years of service. Other firms already have the material on hand and will offer this to the employees for home study, which will save you hundreds of dollars.

If they won't help out, you should try getting a loan or saving up the amount that is required.

Getting MCSE certification has its advantages. For one, it will be easier for you to find a job since almost every company needs to have someone checking on the computer network at all times. This will also allow you to demand more money should you decide to work for someone because you have a document in your hand saying how good you are.

For those that want to have a career in the IT industry, it is best to get MCSE certification. After all, most companies in the United States use this unlike those in Europe that are more familiar with Linux.

Leave Our Overseas Care Workers Alone!

Employing foreign workers has become a necessity in the Care Industry, which would grind to a halt without them.

Following my last article The Care Industry Still Needs Foreign Workers we have received an overwhelming response from Care Managers and owners up and down the country.

The vast majority of calls and letters we received agreed that the industry depends upon foreign workers from outside the EU.

One home owner in the learning disability sector said that he would not have been able to expand his business without his Filipino and Indian staff: they are the backbone of our business and we couldnt operate without them he said.

Several managers were appalled that the Home Office had become much stricter when dealing with new Work Permit renewals for overseas care staff. Some had even lost staff due to Work Permit refusals.

All agreed that they should have the right to choose where to recruit their staff from and be assured that their existing Work Permit holding staff will be safe from future Home Office policy changes.

The message to the government is clear:

Leave our overseas workers alone!

What Documentation is required for Overseas Workers?

I am asked this question several times a day. Although we have covered this issue in previous articles, I thought it might be helpful to give you the following guidelines, prepared in conjunction with Martin Wigg for the Hertfordshire Care Providers Association (HCPA), on the documentation an employer should have for an overseas worker.

For the purpose of this exercise lets assume that there are two categories of overseas or Non UK National Staff:

Citizens of the European Union (EU), European Economic Area (EEA) and the new Accession 8 Countries (A8).
Citizens of Countries outside the EU

EU/EEA Citizens are permitted to live and work anywhere within its borders, including the UK. When recruiting EU Citizens the same documentation of identity as for UK Nationals is required eg: Passport, Original Photograph, Birth Certificate, etc. If you are in any doubt, call the Home Office Employers Help Line on 0845 010 6677.

Citizens of the A8 former eastern bloc nations, such as Poland and Latvia, are permitted to work in the UK subject to certain conditions. People who have worked here for less than 12 months need to register with the Workers Registration Scheme (WRS). For further information visit Useful Websites at http://www.immigrationmatters.co.uk.

In respect of non UK staff from outside the EU, in addition to the usual references and checks, the following documents should be examined, and copies taken for record purposes:

A valid Passport
A valid Work Permit issued by the UK Home Office
A Visa (or Leave to Remain) stamp issued by the UK Consular Office in their country of origin or by the UK Home Office.
CRB & POVA Checks

A Work Permit is not sufficient on its own and is applicable to one employer.

The Expiry date of the Visa should be checked to ensure that it will remain valid for the period worked. If a Visa or valid Leave has expired the person should not work and will normally have to leave the UK.

If a Work Permit holder changes jobs they must apply to the Home Office for a new Work Permit and Further Leave to Remain. It is illegal to employ a NUKN citizen (Non EU/EEA) unless they hold either a valid Visa or Leave to Remain Permit.

In respect of Students the following documentation is required:

A valid Passport

A valid Visa

Letter from the Home Office granting them Student Status. This will be time limited.

A letter from the College confirming they are a current student

CRB & POVA Checks

Students may normally only work a maximum of 20 hours per week during their term time. During holiday times they may work in excess of 20 hours. Sandwich courses may allow them to work longer hours if the course is work related.

NB: if students are going to work in excess of 48 hours they must, like all other staff, sign a Consent to Work in Excess of 48 Hours form.

NOTE: All newly recruited staff (including NUKN Staff & Students) must also be treated in accordance with the Care Standards Act and Employment Law, including undergoing Common Induction Standard Training and receiving a Contract/Terms of employment.

The Second Biggest Mistake Freelancers Make

When you first begin your freelance careerwhether as a writer, photographer, web designer, graphic artist or whateveryou have to face the ultimate challenge: getting clients.

So you scour all the marketing books and Internet articles, and you talk with others who have gone before you to see if they have a secret.

Then, one day, you take the plunge. You begin to market your freelance business. You come to realize that if you don't aggressively market, you are going to get nowhere fast. And that's not why you became a freelancer. You became a freelancer because you wanted to call your own shots and you believed you could make more money working for yourself than for someone else.

If you are diligent and determined, your marketing efforts soon begin to pay off. You land a couple of clients. One of them, in fact, may be a big one. It's a large company with tons of work for you to do and plenty of money to give you in return. Best of all, you've managed to impress them.

Welcome to the gravy train!

Well, not so fast. It is extremely tempting to climb onto this gravy train and go along for the ride as long as it will take you. But "putting all your eggs in one basket," or devoting most of your freelance work to just one client, will end in disastersooner or later.

While you are so busy with your big client, you tend to curtail or even stop marketing, so no one else ever learns about you. You eschew smaller clients and projects because you don't have the time and you can make a lot more money with your big client.

Worst of all, the day will come (I promise) that your gravy train will derail. The company will be swallowed up by some bigger, impersonal corporation that doesn't know you and doesn't care. Your job will become so important that the company will decide it needs a full-timer instead of a freelancer. Or your job will become obsolete as the company moves in a new direction. The contact people you have so carefully cultivated over time will gradually move on to other jobs, so you will have no one on your side. Or, heaven forbid, you will make an egregious mistake that just cannot be forgivenand you will be shown the door.

I know this from firsthand experience. For more than 12 years I road the gravy train of a company that handed me thousands of dollars every month. Then things began to change. The company is no longer operating, and my contacts have all departed for parts unknown.

Too many freelancers make the mistake of allowing a large percentage of their income to come from one source. If one client supplies more than 20 percent of your revenue on a consistent basis, you need to find more clients and other sources of revenueas soon as possible.

I've also known people who worked full-time and then decided to freelance almost exclusively for their former employer! Same mistake. Don't let familiarity doom your bottom line. Always be on the lookout for new clients.

(One caveat: In the very beginning of your freelance career, you may have only one or two clients to start, so they will, of course, contribute a large percentage of your income. But after about six months, you really need to have multiple clients and many baskets full of all those golden eggs.)

Writing, recording and performing terrific music is half the battle, but the other half may be even tougher than the challenge of creating a #1 hitthe task of publicizing it. Without publicity, your great opus might very well sit, gathering dust on your closet floor with the other 999 copies you had printed, cased and shrink-wrapped. Getting your name/your bands name out there is quintessentially the most crucial step to: gigs, management, fans, radio, label exposure and rock n roll stardom.

But what if your new found press is not-so-positive? Should you turn away a chance to pimp your band to the masses if the article is entitled, Worst Bands In The History Of Sound or CDs You Wouldnt Listen To If They Washed Up Next To You On A Desert Island? Is all press good press? Press is only as current as its latest issue and soon enough, what feels like the ruin of your band will be replaced by the ruin of someone elses. But, repeat mentions of it on your websites, My Space pages, etc. will keep the negativity alive as long as you refuse to let it go. I realize that your art is precious to you and that youre easily hurt by bad press but continuing to ruminate on it, only serves to keep the wound open way past the original issue date. The energy you put into bad press should be focused onto getting newer, better press for the band. You will remember the bad press long after everyone else has forgotten about it so let it go and move onto more positive things.

No one every said that the music business was going to be all sugar and spice and everything nice, so it should come as no surprise that youll probably garner as much bad press as good. Foster the positive publicity as much as you can and chalk up the negativity to a small pothole on your road to success. Its true that it doesnt matter what they say as long as theyre talking about you so be thankful for the free PR, take the high road, and let the bad reviews roll by your band and into oblivion where they belong. Its unrealistic to think that you can get everyone to like your music, so make it your goal to get everyone to remember your name.

Long before the star-studded, over-hyped Disney film Pearl Harbor, film creators gave us a much better picture of the events surrounding December 7, 1941. Often shown on the History Channel, Tora! Tora! Tora! almost fits the definition of a documentary. The film doesn't employ sideshow tricks such as love triangles (i.e. Pearl Harbor) in order to create audience interest. In fact, few of the actors stand out on their own. Instead, each character plays his part as it most likely unfolded in real life - and the real life version of Pearl Harbor is interesting enough on its own

Tora! Tora! Tora! originates as the Japanese call sign for a full-fledged attack on the American naval installation at Pearl Harbor. When the Japanese Empire continues its expansion throughout the Asian-Pacific realm, the United States institutes economic sanctions. As further divisions threaten to bring about war between the two nations, Japanese admirals convene in order to plan a surprise preemptive attack on the United States Navy in the event that war becomes inevitable.

Among those present is Admiral Yamamoto (So Yamamura) who agrees to carry out the plan, but as a man who studied extensively in the United States, he believes his fellow countrymen underestimate the resolve and the industrial might of the American people. Despite its 1970 release date, Tora! Tora! Tora! remains a wonderful film that can hold its own in the special effects department with any modern day blockbuster. In fact, the older camera technology makes the scenes subconsciously feel more realistic than modern day films such Pearl Harbor. If you're looking for a good, suspenseful movie (and one which teaches history too), then you'll be hard pressed to find a better option than Tora! Tora! Tora! One of the best war films of all time, Tora! Tora! Tora! is a definite must see film.

Pasta has existed since the days of the Roman Empire and remains one of the most versatile cooking ingredients, as no storage room or cupboard should be without it. It can be combined with meat, fish, vegetables, fruit, or even a simple herb sauce to create a mouthwatering and nutritious meal within minutes.

There are as many as 200 different pasta shapes and about three times as many names for them. To cook pasta, first bring a large pan of lightly salted water to a boil. Add the pasta and one tablespoon of olive oil, but do not cover the pot or the water will boil over. Quickly bring the water back to a rolling boil and avoid overcooking. When the pasta is tender, but still firm to the bite, drain and toss with butter, olive oil, or your prepared sauce. - Fresh filled pasta: 8-10 minutes
- Dried unfilled pasta: 10-12 minutes
- Dried filled pasta: 15-20 minutes

Defining the Traits of a Successful Leader

What makes a good leader? For starters, leaders don't wait for other people to give them permission to do something. They just do it. Leaders accept responsibility for the choices they make in life. They don't get sucked into the "victim mentality" syndrome, which is characterized by a persistent desire for people to blame others for their poor choices. Bottom line: Leaders realize that the decisions they make are all theirs, and thus take full responsibility for any resulting failures.

In the world of business - especially network marketing and direct sales - leadership is the defining ingredient that separates the mediocre from the superstars. It's the act of persuasion. It's getting people to see new perspectives and do things they normally wouldn't do. It's about setting your ego aside and having the passion and charisma to get people to follow you. Leaders don't follow. They just do.

Can an average person become a leader? Yes, most certainly.

People can transform themselves and make huge strides in leadership abilities just as they do in other areas of personal development. It starts with inner self-leadership and expands outward to influence and move others around you.

Leadership is about self-direction and self-control and shows in what "we do." Become the right kind of person (passionate, responsible, doer, believer) and others will flock right into your lap, and not until.

But, like other areas of self improvement, it's no easy task, because man's natural instinct is mediocrity. Yes, mediocrity. It would be nice if we could all become leaders by simply following a few simple steps. But the path to leadership requires finding our own way. The direction we take will differ for each of us. However, there are a few key traits we can focus on.

No Fear But Fear Itself

So what is it that keeps us from pursuing opportunities, leading others, taking action and doing what we really want to do? One word: FEAR. Fear of rejection. Fear of people.

Fear of trying new things. Fear of not being perfect. Enjoying our comfort zone (average).

It's almost as if the fears we have actually begin to become like bondage. Ever feel that way? You can usually tell who lives in fear. They usually wear them on their sleeves. Ever hear people say:

"I would try that but I just don't have time." "That program will never work. I already tried networking and it didn't work before." "I would listen to you, but that sounds like some pyramid scheme."

The list of the doubts and fears that roll off people's lips is endless. Stand in any line with people, on the bus or at work, and listen to the idle chatter. You'll hear people's fear in their everyday conversation. "Oh, I hate working here, but the job market is just too difficult to try and find something else." If you want to be a great leader, learn to conquer your fears.

Embrace Failure

Study the histories of all great leaders and you'll see lives littered with failures. It's called "failing forward fast." And it's one of the top traits of successful leaders. That's why it takes guts to be a leader. Many people are scared of failure and don't like to tackle the tough issues. (Then again, not everyone wants to be a leader, right.) It requires being decisive and a willingness to take chances. And yes, to FAIL - but to accept it, learn from it and move on.

Become A Believer

Children need to "see it" in order to believe it. Leaders believe it BEFORE they see it. Here's something to try: Try doing what you say you will do. (Remember, the odds are against you).

When faced with problems and obstacles, choose to learn from the experience and turn them into an opportunities. Don't whine and complain about yesterday's defeats. (It's counter-productive and you look like a fool next to Thomas Edison). Key: Until you're able to wipe out the past mistakes from the movie projector that keeps playing in your head, you'll never be able to move forward.

Leaders believe in themselves and believe in abundance.

Take Some Risks

Leaders are the ones willing to make huge sacrifices in time, money and family in order to achieve their goals. Risk is the price you pay for success. You must carry the burden and have the backbone to make decisions that are not popular. Be internally directed, not "socially fit." You'll never achieve wealth and success as long as you care what other people thing of you. Yes, leadership has a price, (risk) but it also offers tremendous rewards.

Don't Follow The Crowd

Here's a crucial defining trait of a leader: they don't follow the crowd. Nothing great was ever accomplished by a crowd. A crowd merely blows with the wind, like scattered leaves. Crowds have no purpose and end up nowhere. It takes courage to go against the crowd and be a true individual, but it's the most exciting, exhilarating thing we can do. It's also the most difficult and frightening. That's why success is so elusive and so rare.

Take Ted Turner of CNN fame, for instance. He said, "Lead, follow, or get out of the way!" Was Turner moving with the herd? No, he was the captain, not one of the deck hands.

Most people live in a state of self-consciousness. They go to work, support their families are active in their communities and are good citizens. Their egos are socially supported which means they go with the flow. Thus, because they are moving with the herd and moving with the group's collective consciousness, they struggle. It's never their goals, their vision or their choices. Most people rarely, if ever, move into the direction of freedom and true self expression.

The leaders are your freedom fighters. These are people like Winston Churchill, Henry Ford, Margaret Thatcher, Thomas Jefferson, Bill Gates. Leaders usually affect thousands of people. Anytime you share your energy with enough other people, BAM, success follows. The problem though is that the notion of prosperity scares many people because they were taught that money is evil, or are afraid of the responsibility that comes with success.

While the world continues to change, the traits of good leaders remain constant. The principles are timeless. Once you learn how to liberate the leader within you, extraordinary things happen.

How Do I Know If I Am Having a Heart Attack?

Most patients have symptoms prior to an actual heart attack generally a tightness behind the breastbone, which generally is caused by an increase in heart rate. The coronary arteries fill in between heart beats therefore anything which increases your heart rate will reduce the time your coronary arteries have to provide blood flow to the heart muscle- the myocardium. Once the heart muscle is deprived of oxygen it becomes irritable leading to palpitations and, if not relieved, death of the muscle.

Surrounding the dead muscle is a variable sized area of heart muscle at risk of dying because of inadequate oxygen supply this condition is known as myocardial ischaemia.

The heart rate is under the control of the autonomic nervous systems, which, in the case of the heart, is driven by adrenaline the so-called flight or fight hormone.

Adrenaline acts at various sites of the body

- On the sinus node in the heart which controls rate to increase heart rate
- On the bronchial tubes in the lung to cause them to dilate allowing greater flow of air into the lungs
- On the small arteries in the skin and gut to constrict (narrow) reducing blood flow to these areas and making more blood available to the muscles- to fight or fly
- Increases the blood sugar by increasing breakdown of glycogen to glucose, important food for the muscles to contract better.

Whilst increased adrenaline production is common with exercise it also occurs in states of shock, fear, or anger producing gooseflesh and sweating (remember the first time you saw the shower scene in the Hitchcock Movie Psycho).

As well as increasing the heart rate adrenaline may cause cardiac irregularity, increased blood pressure, or palpitations. It is this irregularity which may lead the heart to chaotic beats and finally fibrillation and death.

This is the first symptom of a heart attack in about 20% of people sudden death the victim just collapses to the ground. Whilst he may gasp a few breaths he has no cardiac output- no blood is leaving his heart- he is pulseless.

This terminal condition (ventricular fibrillation) is very easily treated by first aid measures and cardiac shock- termed defibrillation. Nowadays many areas which have high populations (or as in aeroplanes, confined areas) have people friendly defibrillators available which only shock patients who have rhythms which are treatable by shock. These machines have easy instructions and any bystander who can read can apply the electrodes and save the patients life.

Often people wrongly refer to this cause of death as a massive heart attack- it takes some time (often up to an hour) to die from a massive heart attack. In this case the heart attack causes such significant damage to the main ventricle of the heart as to reduce blood pressure to critically low levels and death results.

Cause of Heart Attacks

The pathological cause of heart attacks is a clot of blood, which forms over an area on the lining of a coronary artery which, is ready for a clot to form this area is termed a vulnerable plaque.

It was previously thought that clots developed on the lining of arterial walls that had become calcified and thickened - it is now known that rarely are these areas the sites of clots.

Calcification and deposition of cholesterol within the arterial wall reduces the lumen size and therefore flow of blood through this section of the artery is likewise reduced. This reduced flow may be sufficient to provide enough oxygen to the heart muscle during rest or light work. However once the heart is required to work harder the blood flow is insufficient to provide enough oxygen to the heart muscle and symptoms follow.

Angina- Pectoris and Decubitus

When the heart muscle receives insufficient oxygen it produces lactic and pyruvic acids and these produces the tightness in the chest known as angina the type of angina which occurs during exercise is termed Angina Pectoris.

The obvious treatment is to reduce the heart work by ceasing whatever activity has caused the heart rate to rise.

Whilst typically angina pectoris occurs as tightness in the chest it may move up into the throat or down one or both arms. Occasionally it may only occur in the teeth or in the back between the shoulder blades. Patients may describe it as a weight on my chest or constriction. Occasionally the only symptom may be acute breathlessness.

The cardinal feature is the relation to increased heart work and its relief by stopping activity. In addition one should use a trintrin preparation. If pain is present use another dose after five minutes or so and repeat twice.

The treatment of angina is beyond the scope of this article however drugs which reduce the work the heart has to do (like putting a governor on your heart) are most effective and as they block the action of adrenaline are termed betablockers.

Drugs, which dilate the coronary arteries (trintrins), are also helpful. These come as patches, tablets for sucking, sprays for inhalation or as solutions for intravenous use in hospital. These should be used only when the tightness is present and not when you feel giddy or weak as they will reduce blood pressure and may cause you to faint. These also dilate all arteries and therefore may cause headache.

Trintrins are particularly dangerous when used in conjunction with some of the newer drugs for erectile dysfunction eg viagra and these must not be used within 24 hours of each other

Spasm of the coronary arteries may also cause reduction in blood flow to the heart. Everyone has some degree of spasm within their bodies and angina is generally due to mixture of both atheroma and spasm.

However angina, which occurs at rest particularly at night around 2.0am waking the patient, is due to vasospasm and is termed angina decubitus.

The symptoms are similar to angina pectoris but in this case it is spasm of the artery and the treatment is not betablockade therapy but a group of drugs called calcium antagonists. These drugs relieve the spasm of the artery.

Unstable Angina

Should the pain be prolonged and not relieved by rest and trintrin therapy the term unstable angina is used.

In this case the pain is more severe and often associated with an increase in sympathetic overactivity. Symptoms such as sweating, nausea, vomiting, palpitations, faintness or a feeling of wanting to faint (presyncope), or even syncope are common.

This is a medical emergency and an ambulance should be called without delay, as it is impossible to differentiate unstable angina from a heart attack without electrocardiograph and blood studies.

If you believe that you may have this syndrome whilst waiting for the ambulance chew a whole aspirin tablet.

This condition is generally associated with a clot being formed within one of the main coronary arteries and hospitalisation is necessary so that a definitive diagnosis may be made and urgent treatment to protect the heart from damage instituted.

Differential Diagnosis

Angina is often confused with indigestion and many patients rationalise that whilst they may consider the pain could be angina it simply cannot happen to me and it is probably only indigestion or just wind.

One eminent Professor of Cardiology writing in one of the journals described how he waited for eighteen hours before going to Hospital as he thought it was simply bad indigestion. He finished up with bypass surgery!

Indigestion is generally in the pit of the stomach burning in nature, often relieved by belching or by drinking a glass of milk or alkali. It has no relation to exercise, rarely referred to the arms or back, and may come up into throat as a burning sensation. Trintrins have no effect.

Hiatus hernia or oesophageal reflux may mimic vasospastic angina as they both occur at night when the patient is prone.

Silent Ischaemia

Some patients, generally those with high pain thresholds, may have myocardial ischaemia which is entirely asymptomatic.

Rarely patients may be found with classical electrocardiographic changes of a heart attack without remembering anything remotely like the symptoms, which are noted above.

On a treadmill exercise test these patients may develop changes on their cardiograph, which are diagnostic of myocardial ischaemia. They may develop high blood pressure or more dangerous, a fall in blood pressure during exercise without any actual pain.

These patients are very difficult to treat, as, unlike most of us, they do not get chest tightness of pressure before they develop dangerous cardiac abnormalities. These people require invasive or nuclear studies to ascertain the workload that causes trouble. They can then be warned to control their work level.

Heart Failure and Cardiac Irregularities

Patients in their senior years may develop heart failure without any pain or tightness. The Electrocardiographic changes of a previous heart attack are found on routine examination.

Atrial fibrillation, a not uncommon rhythm disturbance, often has myocardial ischaemia as its underlying cause, particularly in the elderly. This irregularity results in a reduction in the output from the left ventricle (stroke volume). As blood continues to fill the ventricle from above the lower output results in the heart becoming distended with blood.

Heart muscle like all other muscles when stretched contracts with greater strength thereby increasing the output. The patient is unaware of anything the matter with his heart until an xray is taken showing an enlarged heart . (Compensated Heart Failure)

However if the reason for the reduction in stroke volume remains then eventually (when the heart cannot enlarge further) pressure within the ventricle rises and is passed backwards through the atrium and on into the pulmonary arterioles causing fluid to move into the air sacs of the lungs and the patients notices increasing shortness of breath breathlessness. (Decompensated Heart Failure)

Summary

- Never take chest pain lightly - you might wake up dead !
- Learn a plan and keep to it.
- It is only at an Emergency Centre that you can get immediate diagnosis.
- Most Heart Attacks who are destined to die do so within the first few hours. Dont Delay
- Special Treatment to reduce the residual damage to your heart is most effective when given early. Particularly within three hours of the onset of pain.
- You will not be laughed at for going to the Hospital with indigestion
- Remember no one can exclude a Heart Attack from a Severe anginal pain without ECG and blood tests

Is your cat getting fat, lazy, and bored?

Cat toys are a great way to keep your cats occupied and entertained. They arouse the cat's curiosity and the cat spends considerable time chasing, mangling, twisting, and playing with the toys. The toys that excite them most are usually those that make sounds, swing on strings, or are easy to catch. Cats have a limited attention span, however. It therefore makes sense to rotate out the toys on a weekly to monthly basis. Otherwise your cat will lose interest.

Toys need not be bought from the pet stores alone. You can create an endless number of cat toys at home. There are a number of household objects that may appeal to your cat's fancy; these include toilet paper rolls, plastic milk bottle rings, plastic drinking straws, aluminum foil balls, Popsicle sticks, pipe cleaners, cardboard boxes and even walnuts!

You should, however, carefully examine each item, and reject the ones that have sharp edges. You should also avoid small objects that could be swallowed, objects that tear easily, or those which could stain. These can sometimes cause injuries to your cat.

Pet shops, of course, have a variety of toys, and these are generally safer. These include line-based toys (which have an object dangling from a rod or a string for your cat to chase), and wire-based toys (based on the principle that cats love to chase birds). These flying toys make for good fun. Also popular are 'bat and swat' toys, which come in an array of shapes, colors, sizes, and patterns. You can choose from a fluffy pom-pom to a simple Ping-Pong ball. A major advantage with the bat and swat toys is that your cat can use it on its own; you don't have to be around to entertain it.

Catnip toys can also be highly entertaining to cats. However, these toys should never be given to a kittens less than four weeks of age, as catnip can have a hallucinatory effect. Always ensure that the catnip you buy is fresh, otherwise it could lead to serious health complications for your cat. Fresh organic catnip toys can frequently be found at arts-and-craft shows and cat shows.

Some of the most popular toys among cats are those shaped like rats, insects, birds, or other small creatures; these add a hint of realism to the sport and can stimulate even the laziest cat into action.
